About Us

Unite Tennessee is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities. Unite Tennessee is built in partnership with Ballad Health and the STRONG Accountable Care Community. The network is supported by a Tennessee-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Get Started as a Partner

Community Roots

Network partners are now able to send and receive secure, electronic referrals in Delaware, Georgia, Maryland, Ohio, South Carolina, Tennessee, Virginia, West Virginia, and the DC area through the Unite Us Platform, which ensures communities and individuals can seamlessly access much-needed resources and services across state lines.

It was so amazing to be able to click ‘emergency food’ and send the referral. There was no form, no fax. It’s not hard to refer people, it’s just the click of a button. That’s what I love about Unite Us. After the referral was made, I was able to confirm she was served by tracking her outcome through the software. Being fast is key, because right now, with COVID, every call is critical.

Our staff was BLOWN AWAY by the quick responses to their referrals - they didn’t have to do any of the typical back and forth to get updates on the referral. In one case in particular, a partner couldn’t accept a referral for various reasons. But, instead of simply rejecting it, they sent the referral on to another organization that was able to help. This was so helpful, warm and personal!

This has been, so far, the greatest breakthrough for the entire community. Other than just sending people to services and to providers, Teenagers Plus has now gained a new momentum in the community in terms of creating linkages.

By partnering with the Unite Us network, Ballad Health can truly integrate social care into our comprehensive system of healthcare. This allows for us to work in a more connected, complimentary way with our community partners, while elevating our ability to positively impact the communities we serve. It also allows us to utilize best practices in serving our vulnerable populations, and focus on being more culturally competent and equitable. These are very important commitments for Ballad Health and this is foundational to our journey of becoming a Community Health Improvement System.

Here in Northeast Tennessee and Southwest Virginia, we could not be happier. Health and social care providers really like how easy Unite Us is to learn and how it streamlines the referral process from beginning to end, facilitates patient/client consent, and saves time and effort while improving quality of care. We are working closely with the Unite Us community engagement managers to promote this solution as part of our No Wrong Door initiative, and the spirit of community and collaboration could not be better.
